Summary Pediatric Heart Failure therapies

  • Diuretics for fluid overload
  • ACE inhibitors (or ARB if intolerant) – Recently Entresto (accepted by FDA in children since 2020)
  • Aldosterone antagonists for symptomatic HF
  • Beta blockers (not in acute HF)
  • Digoxin por persistent symptoms (or after acute HF, previous to start beta blockers)
  • Aspirin if LFEV< 30 % or Anticoagulants if thrombus
  • Amiodarone if Non susbtained VTach is detected
  • ICDs for low EF and risk factors (Class I as Secondary prevention)
  • CRT for dyssynchrony, low EF (teenagers)
  • Milrinone with inotropic escalation for severe HF (Repetitive Levosimendan infusions in European hospitals are used, not approved FDA)
  • Mechanical ventricular support and Heart Transplant
  • Experimental treatments (Sterm Cell infusion)
