Summary Pediatric Heart Failure therapies
- Diuretics for fluid overload
- ACE inhibitors (or ARB if intolerant) – Recently Entresto (accepted by FDA in children since 2020)
- Aldosterone antagonists for symptomatic HF
- Beta blockers (not in acute HF)
- Digoxin por persistent symptoms (or after acute HF, previous to start beta blockers)
- Aspirin if LFEV< 30 % or Anticoagulants if thrombus
- Amiodarone if Non susbtained VTach is detected
- ICDs for low EF and risk factors (Class I as Secondary prevention)
- CRT for dyssynchrony, low EF (teenagers)
- Milrinone with inotropic escalation for severe HF (Repetitive Levosimendan infusions in European hospitals are used, not approved FDA)
