Arrhythmias in Pediatric Heart Failure: EKG abnormalities

  • Most common findings of sinus tachycardia, LV hypertrophy, ST-T changes (see EKG below strain pattern), myocardial infarction patterns, and 1st degree AV block

  • In the end-stage of HF the risk of ventricular arrhythmias and sudden death (SD) are increasing. Holter and stress test should be done during the follow-up to establish the SD risk.
  • In some cases the EKG is specific of some entity:
    • DCM: left bundle branch block and left atrial enlargement
    • RCM: biatrial enlargement and RN hypertrophy due to pulmonary hypertension
    • In infants with acute episode of HF and infero-lateral Q waves, ALCAPA (anomalous origen of left coronary from pulmonary artery) has to be ruled out
  • In some cases the arrhythmia is the cause of HF: Ectopic atrial tachycardia, Atrioventricular block or Ventricular pre-excitation
